Plain X-ray Assessment of ACL Insufficiency



Abstract
Dynamic assessment is a well known technique used in the evaluation of knee joint instabilities. Until now, plain radiographs has played a limited role in the evaluation of ACL(Anterior Cruciate Ligament) insufficiencies.
This study was designed to introduce a new procedure of radiographic assessment of ACL insufficioncy.
Lateral radiographs of knees in maximum extension were taken in 18 cases of ACL insufficient knees, and these were compared with those of twenty normal knees.
On the lateral view of the knee, two lines were drawn along 1) the anterior margin of femur and 2) Blumensaat¢¥s line. Tibial translation was defined as distance between anterior tibial cortex and the intersection point (FR point) of the two
lines, F-line and R-line. Tibial displacement was calculated as a percentage value of tibial translation divided by the tibial width of the level.
The tibial displacement was 4.3¡¾¡¾5.0% in normal knees and 18.0 4. 5% in knees with chronic ACL insufficiency. The difference was statistically significant (p=0.30000).
